New Eyes works with social service agencies in one of two ways. Typically, an agency will complete an Agency Application Form (PDF) on behalf of an individual client and send the completed application to New Eyes for review. If the application is approved, New Eyes will send the voucher to the agency within a few weeks for transmittal to the client. In addition, New Eyes has developed a network of agencies throughout the country that periodically receive a batch of vouchers for direct distribution. These agencies review client applications based on Voucher Distribution Guidelines developed by New Eyes.
It is important that agencies and school nurses help clients locate dispensers in their local area who are willing to participate in our program and accept the price guidelines as full payment. Click here to locate participating dispensers in your area. |
