Providing Glasses in the US How to Apply for Assistance

Individuals can apply for a voucher for new eyeglasses through one of our participating social service agencies, a school nurse, or directly to New Eyes. Click here to request information about participating agencies in your area. Click here to obtain an application form to apply directly to New Eyes (PDF).

To apply for assistance, complete the application form and mail it and a copy of your recent eyeglass prescription to:

Your application form will be reviewed, and if approved, you will receive a voucher in 3-4 weeks. Click here to receive information about the status of your application.

Applicants from Kentucky

Clients in Kentucky receive their vouchers through Kentucky Homeplace, a program that links medically underserved rural residents with free or low-cost health services.

If you live in Kentucky, you must request an application from Kentucky Homeplace by:
  • calling the KHP representative in your county, or
  • calling Fran Feltner at 1-800-851-7512 ext. 83654.
